Your card, your health index and your emergency data

For you, a dependent person or an animal in your care.

1

Create the card

It is the unified health identifier of a person or an animal. Other health identifiers can be linked to the same card.

2

Your health index

It works like a book index: it contains links that show where the real data is held. Verified organisations can update it.

3

Prepare emergency data

Create data for yourself, a dependent person or an animal. Fundación UNID's backup service lets you link it to your index.

Your family can follow your journey through the hospital

People you authorise can see whether you are in emergency, having tests or on a ward as the hospital updates the information.

Three situations where SOSChain helps

For police, healthcare professionals, first responders and authorised people.

1

The person does not respond

A police officer, healthcare professional or first responder assists a person who is unresponsive or disoriented. Using the SOSChain identifier, the professional can start a private call to the person's emergency contacts.

2

Only the animal's QR is available

If the person has no card, the accompanying animal's SOSChain QR can be used to start a private call to authorised contacts without revealing phone numbers.

3

The family is waiting for news

Authorised people can follow emergency admission, tests and ward transfer, and receive communications and calls as the service updates the journey.

Send a voluntary report without sharing your identity

During an epidemic or pandemic, the person chooses whether to send it to Fundación UNID or another participating organisation. The anonymous statistical report remains separate from the card and clinical record.

It is always optional. The person chooses whether to send it, what information to contribute and which participating organisation receives it.

What are they used for?

Anonymous reports are grouped by area, approximate date and clinical code. They are compared with official statistics to help spot trends during epidemics and pandemics.

Statistics are compared, not people.

Your identity stays out

Each report receives a new code that cannot link it to earlier reports. It does not include:

  • Name or card number
  • Complete clinical history or files
  • Clinic or professional identity
  • Free text or a code that can follow the person
SOSChain does not create a Digital Twin from these statistical reports.
